In signing this form, I certify that the above information is true and complete to the best of my knowledge. I acknowledge that yoga may be physically strenuous, and I voluntarily participate with the full knowledge that there is a risk of personal injury or property loss. I acknowledge that I voluntarily participate in activities that may take place outside, in the Florida sun, and in the backwaters and canals and ocean. I agree that Yoga Lab LLC is in no way responsible for the safekeeping of my personal belongings while I attend class. I waive and release Yoga lab LLC and its teachers and staff for any and all liability and responsibility from injury, accident, illness, legal and medical fees sustained now or in the future resulting from my participation in any activity offered by Yoga Lab LLC. I understand that I am giving up my right to sue or make any claims of any kind whatsoever against Yoga Lab LLC for any personal injury, property damage/loss or wrongful death whether caused by negligence or otherwise. I authorize Yoga Lab, LLC and those acting pursuant to its authority to (i) record my likeness and/or voice, audio, photographic, video, digital, electronic or any other medium; and (ii) use, reproduce, exhibit, edit, modify and/or distribute these materials via print publication, video or online, on our website or social media for promotional, advertising, educational, and/or other lawful purposes. I release and waive my claims or rights of compensation or ownership regarding such uses and understand that all such recordings shall remain the property of Yoga Lab, LLC. I acknowledge that I am signing this agreement voluntarily and intend my signature to be a complete and unconditional release of liability to the greatest extent allowable by the law.
